Pre-note: before anyone goes on with EA *-licking fan-boy abuse, save it. I have issues too with FIFA 11 and I've raised them appropriately, so under no circumstances is this thread a grovel. ALSO, I'm open to correction, so please feel free to discuss or debate, however with factual or constructive information, not uneducated guess-work or abuse. This post can and may still be updated at any stage.
As per title, there is a lot of blame been thrown on EA's servers regarding disconnections in H2H matches, but very little logic and/or justification in doing so. So having been involved in a lot in various discussions and threads in these forums to date, having also very admittedly and incorrectly bad-mouthed EA's servers over the years for previous FIFA titles, and having seen so many related problems identified by the following (including my own issues earlier during the summer), I thought I'd clarify everything for everyone experiencing connection issues in H2H mtches, regardless of whether they/you are in need of a friendly reminder check-list or indeed a layman's term explanation
Getting straight to the point; if you’re playing online head-to-head matches, they are peer to peer. That means EA's servers determine your location and match you up with another user based on a variety of factors. Once connected, everything is direct between both players. If disconnections occur during a head-to-head game, this is probably due to a connection problem for one of the two players outside of EA's control, or because one of the players has disconnected or interrupted their connection deliberately.
Online Team Play (OTP) and Clubs are a different story. It’s fair to say EA didn’t have the best year in terms of stability for OTP and Clubs, so they've worked hard on making that better for FIFA 12 (see http://forum.ea.com/uk/posts/list/933577.page for more info)
In the mean time and with particular regard to the topic, make sure you follow all the below steps / check-list in order to make sure you've the right set-up in place for H2H matches:
1. LOCAL CONNECTION SET-UP:
Have you set up your connection's port-forwarding and/or DMZ? (see page 15 of the PS3 game manual for required ports and see portforward.com for info on your router - they're all different set-ups). This is required for ALL FIFA online modes. (NOTE - a number of routers do NOT support the enabling of port-forwarding and DMZ together especially when the same IP address is being used. I'd suggest using only one method)
Also, is your router's firmware version up to date? Check your router settings for an update check, or check the manufacturer's website for any driver or firmware updates.
2. P2P CONNECTION, AND YOUR ISP:
Straight off and to the point, the actual game is hosted between the two players, over a direct peer-to-peer (P2P) connection between the peers. While there's a very minute connection from each peer to the EA servers (like a heart-beat) to ensure both peers' connections are live, any lag in the game-play will be as a result of browsing or streaming via the local connection at either peer, or indeed the intentional use of a lag switch, as it is over the P2P connection that there is heavy data packet exchanging occurring between the peers. Any disconnection therefore will be as a result of either peer's connection breaking down (due to consequential lag, local hardware reset or ISP connection loss).
Between your modem/router and your ISP's base stations, is your ISP strong enough of hosting peer-to-peer matches? Even if your connection has super-cool HTTP download and upload speeds that you love boasting about in your forum signatures, could it have high packet loss percentage and/or jitter that affects your peer-to-peer connection which is so heavily relies on with another peer?
Indeed your P2P download programs like uTorrent and Limewire may work just grand on your PC/laptop, but the big difference between them and a head-to-head match is that such programs can afford to get disconnected from any peers from which its downloading data packets as it has several others to connect to and re-download, but in a FIFA head-to-head match once a disconnection occurs, its the dreaded error message "You have lost connection with your opponent".
Find out by testing your ISP connections peer-to-peer capability on www.pingtest.net, and with VARIOUS servers worldwide (much like playing opponents from all over the world) as opposed to testing with just your nearest server.

Remember, I once blamed EA's servers, and for almost 4 years, having seen erratic game play and AI in several online matches, but realised after making changes to my connection and choice of ISP that the faults were with my old connection set-ups and previous ISP's. Since then, the only disconnections I've had are as a result of my router or connection resetting courtesy of some ISP technicalities, but other than that, not one connection dropped. While the only erratic game play and AI I've had is when there has been lag being caused from my opposing peer's poor connection, or my girlfriend streaming Desperate Housewives!!
