This is the Duke_Nukem_Forever_iNTERNAL_DEMO_JTAG_XBOX360-XBLAplus scene release, all credits to XBLAplus for the Demo.
It's also worth mentioning the graphics on the second level are messed up a lot of the time, if you keep retrying the demo should eventually be playable.
